An exciting opportunity has arisen to invite Assistant Practitioners or clinicians working in an equivalent role to apply for these substantive posts to work within the Countywide Community Respiratory Team. Ideally the candidates will have experience of working with Respiratory patients within the community although support and training will be given. The posts cover the Southwest Lincs, Grantham/Sleaford/Bourne plus SE Lincs, Boston/South Holland
Main duties of the job
The successful candidates will work within a supportive environment alongside the Community Respiratory Team and provide high quality care and support patients through their care pathway whilst promoting independence and protecting choice, dignity, privacy and safety whilst supporting senior colleagues and allied health care professionals in the assessment , planning , implementation and evaluation of care .
Post holders will be expected to undertake assessment of the health and psycho-social care needs of patients with a respiratory condition, implement treatment/interventions for patients with exacerbation of respiratory disease in community settings as prescribed by a senior colleague and implement the use of individualised self-management plans to support hospital admission avoidance.
The Community Respiratory Team deliver a number of services including Pulmonary Rehabilitation and an Oxygen Assessment Service and this role will be vital in supporting these aspects of care.

- Demonstrable considerable experience of working within health setting as Assistant Practitioner/ Health care assistant
- Previous experience in working in a community setting
- Foundation Degree in Care Studies or evidence of equivalent study in care
- Care certificate
- Clinical knowledge acquired through training relevant to clinical competencies
- Assessment and clinical skills
- Understanding of the role of the multi-disciplinary team
- Understanding of evidenced based practice
- Computer literate
- Previous experience of the use of SystemOne
